按会话自身的已索引项目提供 CodeGraph 工具，项目由 workspace 目录解析得出。每个项目根经 npx 启动一个 MCP 服务并被该项目下所有会话共享；会话中途建好索引即自动启用，无需重启；harness 退出时受管子进程被回收。
